CoVerified’s Treatment of Your Information
We value the information of everyone who submits a testing result to Current Care Analytics Inc., d/b/a CoVerified (“we”, “our,” “us”, “its” and similar terms).
Please read this Privacy Policy to understand how we treat your information and other information that we obtain in the course of providing our services.
Applicability & Acceptance of this Privacy Policy
This Privacy Policy governs the manner in which CoVerified collects, uses, maintains and discloses information collected from users (a “user”, as well as “you”, “your”, “employer”, “employee”, “resident”, “patient” and similar terms) of the mobile applications and websites (the “Apps”) through which we deliver test result data to users (the “Services”). THIS PRIVACY POLICY APPLIES TO THE APPS AND SERVICES ONLY, AND NOT TO ANY OF OUR OTHER APPLICATIONS, WEBSITES, OR TO ANY OTHER PRODUCTS OR SERVICES OFFERED BY US.
By using the Apps and the Services, you signify your acceptance of this Privacy Policy and our related Terms of Service.
If you do not agree to this Privacy Policy, please do not use our Apps and Services.
Changes to this Privacy Policy
We may, in our sole discretion, revise this Privacy Policy at any time and from time to time. Your continued use of the Apps and Services following the posting of revisions to this Privacy Policy will be deemed your acceptance of such revisions. We encourage Users to frequently check the email account that they use in connection with the Apps, as well as this Privacy Policy, to stay informed about any changes to this Privacy Policy. You acknowledge and agree that it is your responsibility to review that email account and this Privacy Policy periodically. The date at the bottom of this Privacy Policy will be updated for each revision.

Information that We Collect
We may collect all or any of the following information in the course of providing the Services (collectively, “User Information”):
“Health Information” is all of the information that can be derived from the testing results you provide to us for in connection with the Services. Please see the “Health Information” section below.
“Personal Information”, which you provide us when registering for or using the Apps or Services, such as your name, billing and shipping addresses, email address, phone number, date of birth, user ID and password, and payment information.
“Usage Information”, such as the browser name, the type of computer or phone a User is using, technical information about a User’s means of connecting to our Apps, such as the operating system and the Internet or mobile service providers utilized, and other similar information.
“User-Generated Content”, which includes all information generated by a User and shared with CoVerified, either privately or through public forums, on the Apps, such as messages, photos, audio, video, texts, data and other visual or audio content, as well as email addresses of those with whom you share your Information through the Apps.
“Voluntarily-Provided Information”, which is information voluntarily provided by Users in response to surveys, polls and similar forms through which information may be requested on or through the Apps (other than Personal Information and User-Generated Content). To the extent that any of this information includes information concerning family members or others, you should ensure that these other parties have permitted you to provide such information, and by providing such information to us, you are representing that you are permitted to do so.
We may collect any of the foregoing information from Users in a variety of ways, including but not limited to when Users visit the Apps, register on the Apps, fill out a survey or other form, purchase our products, subscribe to a newsletter, mailing list or other communication tool, contact us via email, and in connection with other activities, services, features or resources we make available on our Apps from time to time.
Our Apps may use “cookies” to enhance User experience. A User’s web browser places cookies on their hard drive or device (as applicable) for record-keeping purposes and sometimes to track information about them. Users may choose to set their web browser to refuse cookies, or to alert the User when cookies are being sent. If a User does so, some parts of the Apps may not function properly.
Do-Not-Track Signals
Your browser may allow you to set a “Do Not Track” preference. We currently do not respond to “Do Not Track” or similar signals.
Health Information
This notice describes how your Protected Health Information (PHI) may be used and disclosed and how you can get access to this information. Please review it carefully.
Purpose
CoVerified respects the privacy of protected information and understands the importance of keeping this information confidential and secure. This Notice describes CoVerified’s policies and practices for collecting, handling and protecting the PHI of our customers.
As a CoVerified user, your healthcare information, including, but not limited to, your name, address, testing results and health status needs to be collected, maintained and made available so that CoVerified can administer its Services on your behalf.
You have the right to:
Request certain restrictions on the use of your Protected Health Information;
Receive an accounting of disclosures of use of your Protected Health Information;
Amend your Protected Health Information;
Complain to the Secretary of Health and Human Services if you believe your privacy rights have been violated; and/or
Revoke your authorization to CoVerified to use or disclose your PHI.
How We Use the Information that We Collect
Our primary goals in collecting information from and about you are to help us provide, operate, improve, understand, customize, and support our Apps and the services delivered through such. Specifically, we use your information for the following purposes:
Provide You with Services You Request. We may use your information to the purpose of enabling you to use the services provided through the Apps. For example, we may use your data to provide you with access to your test results. We may also communicate with you regarding our Apps, including responding to your questions and concerns. For example, we may need to use your contact information for customer support purposes.
Usage Information: We use your Usage Information for a variety of purposes, such as to understand how our Users as a group use the Apps, to improve the Apps, to personalize User experience, and to process transactions. We may also use this information to improve our existing services and the content of our Apps, and research and develop new offerings. Unless otherwise specified and agreed to, these research and development activities will only use anonymous and aggregate data and not your personal information. In circumstances in which your personal information will or may be used for research or development purposes, we will ask for your explicit consent to do so.
User-Generated Content: We use User-Generated content to more effectively respond to your customer service requests and support needs, to improve customer service, to facilitate online message boards and posts in which Users participate and/or to which they contribute, and for similar purposes.
Voluntarily-Provided Information: We use Voluntarily-Provided Information to send Users information that they agree to receive about topics that may be of interest to them, and to share User Information with third parties, including for marketing or other purposes as permitted under this Privacy Policy.
If you do not want your information collected, please do not submit it to us. If you have already submitted information and would like to review, correct or remove it from our records, please contact us at info@coverified.us.
You can opt out of our marketing communications at any time by selecting the “unsubscribe” link in the e-mail or contacting us at info@coverified.us. Please note that some non-marketing communications are not subject to general opt-out, such as communications relating to Services we are providing you, sales transactions, software updates and other support related information, patches and fixes.
Use or Disclosure Required for Compliance: CoVerified and its affiliates are expressly permitted to use any or all of the User Information to the extent that it is required or compelled to do so by law, including detecting, investigating and preventing activities that may violate our policies or applicable laws. CoVerified shall be under no obligation to contest a valid order of a court or other governmental body to disclose User Information. No such use or disclose will be considered a breach of this Privacy Policy.
We do not sell your personal information. If you are a Nevada resident who would like to opt-out of any future sale of your information in the unlikely event we change our practices, you may do so by contacting us at info@coverified.us. Your request must include your full name, street address, city, state, zip code, and an email address so that we can contact you if needed to verify your identity and the authenticity of the request.
Disclosure
Unless we expressly inform you otherwise or have your consent, we only share your Personal Information, including Health Information as follows:
We may use other companies to perform services on our behalf as necessary to administer the requested Services and Apps. We share your information with these service providers only as necessary to perform these functions and provide such services, and only pursuant to appropriate confidentiality and security obligations, requiring such third-parties to maintain the privacy and security of your data
As permitted by law with our attorneys, accountants and auditors, your employer, other authorized representatives, or other third-parties, including in to establish or exercise our legal rights and/or to defend against legal claims, or in response to a subpoena or similar investigative demand, a court order, or other request from a law enforcement or government agency or as otherwise required by applicable law.
To government agencies or accrediting organizations that monitor our compliance with applicable laws and standards.
We may share aggregated and non-personal information with third parties for their legitimate business purposes. If we would like to share your personal information for any purposes other than those described, we will obtain your prior consent.
How we Protect User Information
We adopt data collection, storage and processing practices and security measures to protect against unauthorized access, alteration, disclosure or destruction of User Information and data stored on our Apps. Commercial transactions occurring on the Apps are processed over a SSL secured communication channel. However, no method of Internet transmission is completely secure and we cannot guarantee its absolute security.
User Information may be stored in United States and/or in foreign countries. Ultimately, the requirements of governments other than those of the United States may apply to User Information.
Third Party Websites
Users may find advertising or other content on our Apps that link to the sites and services of our partners, suppliers, advertisers, sponsors, licensors and other third parties. We do not control the content or links that appear on these sites and are not responsible for the practices employed by websites linked to or from our Apps. In addition, these sites or services, including their content and links, may be constantly changing. These sites and services may have their own privacy policies and customer service policies. Browsing and interaction on any other website, including websites which have a link to our Apps or websites, is subject to that website’s own terms and policies. With respect to links from the Apps to our other websites (including other CoVerified websites), please refer to the privacy policies on those websites.
Termination of User Account
If you no longer wish to receive the Services, you may terminate your account by sending a request to info@coverified.us. We will remove all of your Health Information from the Apps within 30 days of our receipt of your request. To the extent provided above, however, any Health Information or Personal Information will be available for use on an ongoing basis following removal of your Health Information from the Apps. CoVerified and its affiliates may also retain your Health Information as required by law or internal data retention policies. We will also retain limited Personal Information for internal reporting and compliance purposes, and as otherwise required by law.
Transfer of Information in the Event of Acquisition
Your agreement to this Privacy Policy includes your consent to the transfer of your User Information to any entity that may acquire CoVerified or its assets, either directly or indirectly, and to any affiliate of CoVerified.
Third Party Beneficiaries
This Privacy Policy is between us and you, and does not grant any rights to third parties.
Governing Law; Dispute Resolution
This Privacy Policy will be governed by and construed in accordance with the laws of the State of Massachusetts, USA, without regarding to conflicts of law principles.
If a matter arises that cannot be resolved promptly between you and us, you agree that any disputes arising out of or relating to the Service or this Privacy Policy (including without limitation the validity and scope of the agreement to arbitrate and any disputes with other Users of the Service) shall be resolved exclusively by final and binding arbitration administered by the American Arbitration Association (“AAA”) under the Federal Arbitration Act, and shall be conducted before a single arbitrator pursuant to the applicable Rules and Procedures established by the AAA (for information on the AAA and its rules, see adr.org). You agree that the arbitration shall be held in Boston, Massachusetts, unless the AAA or the arbitrator shall determine that venue in such city is unreasonably burdensome, in which case the AAA or the arbitrator shall select a venue that is not unreasonably burdensome to both you and us. You further agree that, if the AAA shall be unavailable or decline to administer the arbitration, and the parties do not agree on a substitute, a substitute administrator or arbitrator shall be appointed by the court. This provision shall not apply to claims of patent, trademark, or copyright infringement or misappropriation of trade secrets. In addition, you or we may elect to bring an individual claim in a small claims court, but we do not hereby agree to any personal jurisdiction that is otherwise lacking.
You agree that any arbitration shall not permit claims on a class, mass, representative, or private attorney general basis. You further agree that no claims of other parties may be consolidated with your or our claims in the arbitration without both your and our consent. YOU ACKNOWLEDGE AND AGREE THAT YOU ARE HEREBY WAIVING YOUR RIGHTS TO HAVE YOUR CASE DECIDED BY A JURY AND TO PARTICIPATE IN A CLASS, MASS, REPRESENTATIVE, PRIVATE ATTORNEY GENERAL, OR CONSOLIDATED ACTION AGAINST US.
If any part of this Arbitration clause is later deemed invalid as a matter of law, then it shall be severed and the remaining portions of this section shall remain in effect, with the exception that if the preceding paragraph is deemed invalid, than this entire section shall be deemed invalid and the arbitration clause shall be void.
